Job summary

Therapy 2000 in Texas is seeking a dedicated pediatric Physical Therapist to provide high-quality home-based services to children and families. You will perform comprehensive evaluations, develop and adjust treatment plans, and collaborate closely with caregivers and other healthcare professionals to support each child’s growth and comfort.

Requirements include 3+ years in pediatric physical therapy, home-health experience, PCS or similar certifications, and bilingual English/Spanish.

Qualifications

• Experience in a home-health setting.
• Specialty Certifications (example: PCS) preferred.

Responsibilities

What you'll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
