Job summary

Therapy 2000 in Houston's Memorial area is seeking a Pediatric Physical Therapist to join a mission-driven team delivering home-based therapy to children and families.

You will conduct PT evaluations, develop treatment plans, and coordinate care with caregivers and other providers. This role requires 3+ years of pediatric PT experience and bilingual English/Spanish, with access to continuing education, mileage reimbursement, a company laptop, and a company-sponsored 401k.

Qualifications

• Three+ years experience as pediatric PT.
• Experience in home-health settings.
• PCS or similar specialty certification preferred.
• Bilingual in English and Spanish.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Develop home exercise programs with patients’ caregivers.
• Coordinate care between providers and disciplines within the patient’s medical neighborhood.
• Meet weekly patient visit targets and complete chart updates and paperwork.
• Adhere to agency policies, procedures and regulatory requirements.

Qualifications

What you’ll need:

Responsibilities

What you’ll do:

• -Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• -Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• -Develop home exercise programs in partnership with patients’ caregivers.
• -Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• -Complete a minimum number of patient visits on a weekly basis as required by the agency.
• -Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• -Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• -Perform all duties in compliance with agency policies and procedures.
• -Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
