Therapy 2000 is hiring a Pediatric Physical Therapist to serve Texas families in a home-based setting. You will evaluate children, design individualized plans, and coordinate with caregivers and healthcare providers to drive meaningful outcomes.

The role requires an active Texas PT license, reliability, and the ability to adapt to changing patient needs; travel between patient visits is reimbursed, and the program offers comprehensive benefits.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patient needs.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with patients' caregivers.
• Collaborate with other providers and disciplines within the patient’s care network.
• Complete a minimum number of patient visits weekly as required.
• Maintain timely chart updates, progress summaries, call logs, and related paperwork.
• Ensure adherence to agency policies, procedures, laws and ethical guidelines.

Qualifications

What you’ll need:

• A current/active license to practice Physical Therapy in the state of Texas.

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
