Therapy 2000 is seeking a skilled Pediatric Physical Therapist to deliver home-based therapy across multiple Texas locations, including Georgetown, Round Rock, and Pflugerville. You will conduct evaluations, design individualized treatment plans, and collaborate with families and care teams to support children's growth and development.

Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (e.g., PCS).
• Bilingual English/Spanish.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Collaborate with other providers and disciplines to coordinate care.
• Complete a minimum number of patient visits weekly as required by the agency.
• Meet deadlines for chart updates, progress summaries, and call logs.
• Coordinate with agency personnel to maintain understanding of expectations and standards.
• Perform duties in compliance with agency policies and applicable laws.
• Adhere to ethics and regulatory guidelines for home-health pediatric PT.

Responsibilities

What you’ll do:

• -Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• -Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• -Develop home exercise programs in partnership with patients’ caregivers.
• -Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• -Complete a minimum number of patient visits on a weekly basis as required by the agency.
• -Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• -Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• -Perform all duties in compliance with agency policies and procedures.
• -Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
