Job summary

THERAPY 2000 in Texas seeks a licensed Pediatric Physical Therapist to join a mission-driven home-based care team. You will conduct comprehensive evaluations, develop treatment plans, and collaborate with caregivers and other providers.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs
• Develop and implement treatment plans aligned with physician orders
• Develop home exercise programs in partnership with patients’ caregivers
• Collaborate and coordinate care between providers and disciplines
• Complete minimum patient visits weekly as required
• Meet deadlines for chart updates, progress summaries, call logs, and paperwork
• Adhere to agency policies, procedures, and applicable laws for home-health pediatric PT

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
