Qualifications

Responsibilities

• Collect and report patient functioning data and metrics.
• Enter data into the patient care registry.
• Communicate between collaborative care team members and with patients and families.
• Provide direct patient contact to assigned patients and family members.
• Provide direct intervention with patients and families, including crisis intervention when required.
