Children's National is seeking a Case Manager II to support patients and families across inpatient units, psychiatry, ED, and related departments. The role emphasizes comprehensive, compassionate care with strong care coordination and resource navigation.

Qualifications

• Master's Degree required.
• Five (5) years' experience with at least three (3) years in an acute inpatient setting.
• Registered Nurse license in DC required.
• Current certification in Case Management or Utilization Management (Preferred).

• Provide professional case management services focusing on assessing needs, contributing to care planning, and implementing targeted interventions.
• Collaborate with multidisciplinary teams to coordinate services and ensure continuity of care.
