Qualifications

• Master's Degree required.
• Five (5) years' experience with at least three (3) years in an acute inpatient setting.
• Registered Nurse license in DC required.
• Current certification in Case Management or Utilization Management (Preferred).

Responsibilities

• Provide professional case management services focusing on assessing needs, contributing to care planning, and implementing targeted interventions.
• Collaborate with multidisciplinary teams to coordinate services and ensure continuity of care.
• Maintain a focus on follow-up care to support long-term outcomes.
• Engage in community outreach to connect families with essential resources and services.
• Promote evidence-based nursing practice and participate in PI processes.
