Sutter Health's PAMF-Palo Alto Medical Foundation MPD is seeking an Assistant Case Manager III to collaborate with physicians, therapists, and care teams to ensure appropriate outpatient care. You will collect data, report metrics, and coordinate care with families, schools, and community resources in a fast-paced environment.

Qualifications include 1 year of relevant experience, AA/BA preferred, strong communication, and the ability to manage priorities.

• AA in Health field or equivalent experience.
• Bachelor's in social work, counseling psychology, or related field preferred.
• 1 year of recent relevant experience.
• 2 years' of recent relevant experience highly preferred.
• Oral and written communication skills.
• Interpersonal and time management skills.
• Ability to work in a fast-paced environment with shifting priorities and competing demands.
• Ability to work independently with minimal direction.

Responsibilities

• Collaborate with physicians, psychotherapists, consulting psychiatrists, and care team members to ensure appropriate level of care for outpatient services.
• Collect and report patient functioning data and metrics; enter data into the patient care registry.
• Communicate between collaborative care team members and with patients and families.
• Provide direct patient contact to assigned patients and family members.
• Provide direct interventions with patients and families, including crisis intervention and de-escalation when required.

Position Overview

The Assistant Case Manager III collaborates with physicians, psychotherapists, consulting psychiatrists, and other care team members to assure appropriate level of care and coordination for outpatient services. Reporting to the Healthy Young Minds Clinical Manager, this role works closely with patients, families, physicians, the interdisciplinary care team, schools, ancillary services and community resources. Responsibilities include collecting and reporting patient functioning data and metrics, entering data into the patient care registry, communicating between collaborative care team members, direct patient contact to assigned patients and family members, and direct intervention with patients and families, to include crisis intervention and de-escalation where required.

Equivalent experience will be accepted in lieu of the required degree or diploma.

Associate's: AA in Business/Health related field.

Bachelor's in social work, counseling psychology, or a related field highly preferred.

Pay Range is $35.35 to $49.48 / hour
