Qualifikationen

• Associate’s AA in Business/Health related field.
• Bachelor’s in social work, counseling psychology, or a related field highly preferred.
• 1 year recent relevant experience; 2 years may be preferred.

Aufgaben

• Collect and report patient functioning data and metrics.
• Enter data into the patient care registry.
• Communicate between care team members and with families.
• Direct patient contact with assigned patients and families.
• Crisis intervention and de-escalation as required.
