Homegrown Essential Wellness in Smithtown, NY is seeking a passionate Nurse Practitioner to join our growing team. You will evaluate patients across newborns to young adults, develop wellness plans, and provide holistic care in a patient-centered environment.

Responsibilities include comprehensive assessments, diagnosing and treating conditions, prescribing therapies, and collaborating with physicians to ensure optimal outcomes. DEA license and NY NP certification required.

Competenze

• Active NP license in New York.
• MSN or DNP from an accredited program.
• Current DEA certification with NY prescriptive authority.
• Board certification (AANP, ANCC, or equivalent).
• Experience in primary care or wellness-focused practice preferred.
• Strong interpersonal and patient-first communication.
• Ability to work independently and collaboratively in a fast-paced clinical setting.
• Medicaid credentials a plus.

• Maintain accurate and timely documentation in the electronic health record (EHR) system

Requirements

• Active Nurse Practitioner (NP) license in the state of New York


• Master's degree or higher in Nursing (MSN, DNP) from an accredited program


• Current DEA certification and prescriptive authority in NY


• Board certification through AANP, ANCC, or equivalent credentialing body


• Experience in primary care, integrative medicine, or wellness-focused practice preferred


• Strong interpersonal and communication skills with a patient-first approach


• Ability to work independently and collaboratively in a fast-paced clinical setting


• Medicaid credentials a plus
