States HR CO is seeking a Family Nurse Practitioner for an outpatient primary care practice in New York. You will deliver comprehensive care for patients across all ages and work within a collaborative healthcare network to support preventive care and wellness.

Qualifications

Responsibilities

• Provide comprehensive primary care across all life stages in outpatient setting.
• Diagnose, treat, and manage acute and chronic conditions.

• Collaborate with physicians, APNs, nursing staff and other clinicians.
• Maintain accurate clinical documentation using EMR/EHR.
