Homegrown Essential Wellness in Smithtown, NY is seeking a passionate Nurse Practitioner to join our growing team. You will evaluate patients across newborns to young adults, develop wellness plans, and provide holistic care in a patient-centered environment.

Responsibilities include comprehensive assessments, diagnosing and treating conditions, prescribing therapies, and collaborating with physicians to ensure optimal outcomes. DEA license and NY NP certification required.

Qualifications

• Active NP license in New York.
• MSN or DNP from an accredited program.
• Current DEA certification with NY prescriptive authority.
• Board certification (AANP, ANCC, or equivalent).
• Experience in primary care or wellness-focused practice preferred.
• Strong interpersonal and patient-first communication.
• Ability to work independently and collaboratively in a fast-paced clinical setting.
• Medicaid credentials a plus.

Responsibilities

• Conduct comprehensive patient assessments, physical exams, and wellness evaluations.
• Diagnose and treat acute and chronic conditions per evidence-based practice.
• Develop and manage individualized patient care plans focused on overall wellness.
• Prescribe medications, therapies, and lifestyle interventions as appropriate.
• Collaborate with physicians and interdisciplinary teams for optimal outcomes.
• Educate patients on preventive care, nutrition, and wellness strategies.
• Maintain accurate documentation in the EHR system.
