Job summary

Affirmed Home Care is expanding our Pediatric Division and seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled, one-on-one nursing care in a home setting for a 1-year-old girl with a tracheostomy, ventilator, and feeding tube in Fairfield County, CT.

You will join a dedicated team to deliver safe, family-centered pediatric care, with flexible per diem shifts including day, evening, overnight and weekends, and strong onboarding and ongoing education support.

Qualifications

• Active Connecticut LPN license.
• Current BLS certification.
• Minimum one year of pediatric nursing experience preferred.
• Experience with tracheostomy, ventilator, and feeding tube care strongly preferred.
• Home care, hospital, or acute-care pediatric experience preferred.
• Authorized to work in the United States.
• Valid driver's license preferred.
• Physical examination completed within the past 12 months.
• Current TB screening, including PPD or QuantiFERON completed within the past year, or chest X-ray documentation when applicable.

Responsibilities

• Provide one-on-one pediatric nursing in the home.
• Support a pediatric patient with tracheostomy, ventilator, and feeding tube care.
• Ensure patient safety, comfort, and well-being in the home setting.
• Collaborate with family and clinical team for care planning and updates.
• Participate in onboarding and ongoing clinical education.

Affirmed Home Care is expanding our Pediatric Division and seeking compassionate, experienced Licensed Practical Nurses (LPNs) to provide skilled, one-on-one nursing care to a 1-year-old pediatric girl with a tracheostomy, ventilator, and feeding tube in the comfort of her home in Fairfield County, Connecticut.
