Job summary

Advocate Health is seeking a pharmacist for Carolinas Medical Center - LCH: Pharmacy to oversee pharmaceutical care and optimize patient outcomes. You will educate patients, review orders, and support safety and formulary processes in a fast-paced hospital setting.

The role requires licensure in the appropriate state, immunization certification, and credentialing. A doctoral or B.S./D.Pharm with ACPE/NABP credentials is expected.

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non‑vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs therapy regimens.
• Educates patients and caregivers on medications and adherence.
• Evaluates and implements patient-specific regimens from biomedical literature.
• Interprets medication orders and prescriptions accurately.
• Supports medication safety and quality improvement activities.
• Manages formulary adherence and non-formulary acquisitions.
• Precepts pharmacy students and trains staff.
• Ensures compliant storage and handling per regulations.
