Job summary

Qualifications

• Licensed in state of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification may be required.
• Credentialing and privileging required based on area of practice.
• Pharmacy degree from ACPE-accredited school or NABP FPGEC certification.
• Strong communication and organizational skills.

Responsibilities

• Oversees pharmaceutical care; evaluates literature and designs regimens.
• Educates patients and caregivers about medications and safety.
• Designs, monitors, and implements patient-specific regimens.
• Reviews and interprets medication orders and prescriptions.
• Participates in medical emergencies and safety/quality activities.
• Supports formulary adherence and medication-use processes.
• Prepares, dispenses, stores and handles medications per policy.
• Precepts students and mentors pharmacy teammates and residents.
• Ensures compliance with policies, procedures, and regulations.
• Provides care appropriate to patient age-specific needs.
