Job summary

UNC Health's Pediatrics Pharmacy team in Chapel Hill seeks a pharmacist to provide patient-centered care for pediatric patients in PCICU, PICU, and pediatric cardiology. The role involves day shifts (7am-3:30pm) with some evening coverage and includes mentoring pharmacy students and residents.

Qualified candidates will hold a PharmD or BS Pharmacy, be licensed or eligible in NC, and may have completed PGY1/PGY2 pediatrics. Strong initiative, communication and collaboration are essential.

Qualifications

• Qualified candidates will have earned a PharmD or BS Pharmacy degree and must be licensed or eligible to practice pharmacy in the State of North Carolina.
• Completion of a PGY1 or PGY2 residency in pediatrics or an equivalent level of pharmacy experience is preferred.
• Candidates should possess and demonstrate initiative, communication, analytical skills, organization, and collaboration.

Responsibilities

• Ensures timely medication access for service patients; including order review and verification.
• Identifies and intervenes on problems related to the patient’s medication needs.
• Analyzes patient data to provide optimal drug therapy recommendations.
• Performs protocol-driven clinical services such as therapeutic dose monitoring and antimicrobial stewardship.
• Compounds and oversees preparation and dispensing of medications including sterile products.
• Maintains patient profiles within EMR to facilitate communication.
• Complies with formulary and therapeutic substitution programs to optimize therapy and minimize cost.
• Facilitates discharge and transfer of patients including medication reconciliation and counseling.
• Participates in Code Blue situations for pediatric patients and mentors students and residents.
