Zusammenfassung

The Visiting Nurse Association of Texas (VNA) seeks a dedicated Pediatric RN Case Manager to deliver high-quality hospice care in patients' homes across North Texas. You will act as the key clinical link, coordinating with an interdisciplinary team to ensure individualized, compassionate care for pediatric patients.

Requirements include Texas RN licensure, valid driver’s license, CPR, and experience in pediatrics or hospice.

Qualifikationen

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current Texas State RN License and valid Texas Driver's License.
• CPR certification and TB test; Hepatitis consent/declination.

Aufgaben

• Assess pediatric hospice patients comprehensively across physical, psychosocial, and environmental domains.
• Develop and document individualized care plans ensuring continuity and quality of care.
• Provide clinical leadership to Hospice Aides and LVNs and assist in training new staff.
• Advocate for patients by communicating with families and care teams to meet needs.
• Manage admissions and participate in on-call coverage for emergencies.
