Job summary

Visiting Nurse Association of Texas seeks an RN Admissions to perform initial assessments and coordinate patient care in home hospice settings. You will work with an interdisciplinary team to ensure smooth transitions into hospice services.

Requirements include TX RN license, a valid TX driver's license, and 2+ years RN experience with hospice. Travel to patients' homes is required; competitive salary and benefits are offered.

Qualifications

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current Texas State license as a Registered Nurse; valid Texas Driver's License.
• Minimum of two (2) years RN experience in a clinical setting, and one (1) year hospice experience.

Responsibilities

• Perform initial patient assessments and coordinate care for hospice admissions.
• Develop and document comprehensive plans of care with IDT and physicians.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Meet productivity standards (2–3 admissions per day) and participate in on-call rotation.
