Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for CHRISTUS VNA Hospice in San Antonio, TX. You will assume full nursing responsibility for the Plan of Care for hospice patients and collaborate with a multidisciplinary team to promote wellbeing across physical, psychosocial, emotional, and spiritual dimensions.

Requirements include current unrestricted RN licensure in Texas, a valid driver's license, and CPR certification or ability to obtain within 90 days.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to dependable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates daily with patient/family, physician, and IDG members.
• Instructs patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient's condition.
• Function in diverse home settings and with varied backgrounds.
