Job summary

The Staff Pad is partnering with a respected hospice provider in the DFW Metroplex to recruit a compassionate Pediatric RN Case Manager. This field-based role delivers pediatric hospice care in patients' homes, coordinating an interdisciplinary team to provide personalized, dignified care to children and families.

Reporting to the Managing Director - Hospice, the Pediatric RN Case Manager will conduct assessments, plan and document care, and support ongoing quality improvement while maintaining

Qualifications

• Graduate of an accredited Diploma, ADN, or BSN Nursing program.
• Current Texas RN license.
• Minimum one year of pediatric RN experience in a clinical setting.
• Hospice, Critical Care, or Emergency Room experience preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Current TB testing and Hepatitis consent/declination documentation.
• Ability to pass a criminal background check.

Responsibilities

• Conduct comprehensive physical, psychosocial, and environmental patient assessments.
• Develop, implement, and document individualized plans of care.
• Communicate effectively with patients, families, physicians, and the care team.
• Provide clinical guidance to Hospice Aides and LVNs and assist with training as needed.
• Complete pediatric admissions and prioritize patient scheduling.
• Participate in assigned on-call rotations for emergency patient support.
• Maintain accurate documentation and ensure compliance with infection control and agency policies.
• Support ongoing quality improvement and professional development.
• Perform other duties as assigned.

Reporting to the Managing Director - Hospice, the Pediatric RN Case Manager, will provideskilled nursing care to pediatric hospice patients while coordinating allaspects of their care. Working alongside an interdisciplinary team, physicians,and community partners, you'll help ensure every child receives compassionate,dignified, and personalized care.

What We Are Looking For
