Job summary

KidsCare Home Health is seeking a Physical Therapist to provide pediatric in-home physical therapy, evaluating and guiding patients and caregivers to restore and enhance function. The role emphasizes patient-centered care within a supportive, mentor-driven environment and offers a full benefits package including health, dental, vision, disability and life insurance, PTO, 401K, and tuition reimbursement.

Qualifications

• Master’s Degree in Physical Therapy is required.
• Current state-issued professional license is required.
• CPR certification is required.
• Valid state driver license is required.
• Pediatric clinical experience preferred.
• Home health experience preferred.

Responsibilities

• Examine, evaluate, and administer remedial treatments for pediatric patients.
• Provide clinical treatment including screenings, evaluations, direct treatment, and family/caregiver training and counseling.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and home programs.
• Refer patients and family members to appropriate services.
• Consult with physician on patient progress and maintain records.
• Maintain supplies and equipment.
• Participate in Quality Assurance and Performance Improvement program.
• Attend Continuing Education activities and expand scope of service.
• Maintain adequate caseload with productivity.
• Collaborate with others and mentor students/new hires.
• Actively participate in professional organizations.
• Demonstrate initiative and independent judgment.
• Serve as backup provider for other caseloads.
