Thom Child & Family Services in Norwood, MA is seeking a Physical Therapist/PT for Early Intervention to provide home and community-based services to infants and toddlers birth to three and their families. You will partner with caregivers to tailor services, grounded in evidence-based practice.

Qualifications

• Massachusetts PT license required.
• Experience with infants, toddlers and families preferred.
• Strong written and verbal communication and organizational skills.

Responsibilities

• Participate in multidisciplinary team assessments of the client’s developmental status.
• Assess family and child strengths/resources, concerns, and progress.
• Provide child and family centered services for birth to three and their families using parent coaching model.
• Provide service coordination by scheduling, developing, implementing, monitoring, and evaluating the intervention plan.
• Partner with families in the development of IFSPs to identify goals and strategies.
• Facilitate parent-child interactions to support development; help families advocate for their child.
• Complete progress notes, reports, assessments, and correspondence using Thom electronic documentation system.
• Provide individual and group interventions for infants and toddlers with developmental delays.
