Job summary

St. Mary's Healthcare System for Children in New York state invites a qualified Primary Care RN to join our home care team. You will provide nursing services per physician plan, assess needs, and coordinate care across disciplines to support our pediatric patients in their homes.

Qualifications

• Associates degree in nursing; BSN preferred.
• Current RN license to practice in New York State.
• Minimum of one year of nursing experience in pediatrics.

Responsibilities

• Provide Home Care nursing services per physician plan of treatment.
• Collaborate with team to re-evaluate nursing needs and revise the plan of care.
• Document accurately and submit documentation in a timely fashion per program policy.
• Participate in on-call rotation.
• Meet productivity standards and contribute to quality improvement.
