Job summary

Pediatric Home Service (PHS) is seeking a dedicated pediatric nurse to provide skilled nursing care in home and school settings for medically complex children. You will closely monitor patients, administer medications, and support families through education, ensuring care plans are followed and safety is maintained.

Qualifications

RN license required to provide skilled nursing care.
Experience with medically complex pediatric patients preferred.
Ability to document accurately and communicate with care team.

Responsibilities

Provide skilled nursing care in home and school settings
Assess and monitor patient condition and communicate changes with the care team
Administer medications and prescribed therapies
Provide trach, ventilator, airway, and G-tube care as needed
Maintain accurate documentation and support families through education
Follow individualized care plans and maintain a safe care environment
