• Provide skilled nursing care in home and school settings.
• Assess and monitor your patients' condition and communicate changes with the care team.
• Administer medications, treatments and prescribed therapies.
• Provide trach, ventilator, airway and G tube care as needed.
• Maintain accurate documentation and support families through education.
• Follow individualized care plans and maintain a safe care environment.
