Pediatric Home Service, Inc. (PHS) is a pediatric home health provider serving children in home and school settings. We seek RN/LPN licensed nurses to deliver skilled care, education, and support to families caring for medically complex children.

Qualifications

• Active RN or LPN license required.
• Pediatric home health experience preferred.
• Ability to work in home and school settings.
• BLS/CPR certification preferred.

Responsibilities

• Provide skilled nursing care in home and school settings.
• Assess and monitor patient condition and communicate changes with the care team.
• Administer medications, treatments, and prescribed therapies.
• Provide trach, ventilator, airway, and G-tube care as needed.
• Maintain accurate documentation and support families through education.
• Follow individualized care plans and maintain a safe care environment.
