Qualifications

• Provide skilled nursing care in home and school settings.
• Assess and monitor patients; communicate changes with the care team.
• Administer medications and prescribed therapies.
• Provide trach ventilator, airway and G tube care as needed.
• Maintain accurate documentation and educate families.
• Follow individualized care plans and maintain a safe care environment.
