Qualifications

• Graduate of an NLN accredited School of Nursing.
• Associate of Science in Nursing or higher.
• Current Illinois RN license.
• Active BLS Certification.
• SANE - AA or SANE - P Certification: must be enrolled within 6 months of employment (18 months for new grad RNs).
• BSN preferred; ADN nurses must enroll in a BSN program within six to nine months and complete BSN within 30 months.

Responsibilities

• Assess patient needs and develop a care plan with Case Manager.
• Collaborate with Case Manager to develop care plans for specified patients.
• Contribute to development of clinical paths and use care plans.
• Deliver patient care per hospital policy and nursing standards.
• Document patient care in records and ensure continuity from admission to discharge.
• Administer medications as ordered and ensure patient safety.
• Coordinate with multidisciplinary team for patient‑centered care.
• Support discharge planning to ensure smooth transition home.
