Job summary

Thom Child & Family Services in Lowell, MA is seeking a Registered Nurse (RN) for the Welcome Family program to deliver home and community-based services to newborns and their families. The position emphasizes coaching, health education, and referrals, with visits conducted in the family's home or a nearby location.

The RN will perform assessments, track developmental milestones, and collaborate with caregivers and community partners to support infant health and safety.

Qualifications

• Current Registered Nurse licensure in the state of Massachusetts.
• BSN preferred.
• Excellent communication and organizational skills.
• Ability to work independently and as a team member.
• Abide by confidentiality and professional boundaries standards.
• CPR/First Aid required within the first 6 months.

Responsibilities

• Assist and/or complete initial intakes and provide follow-up referrals and/or interagency service coordination as needed.
• The RN will focus on medical history, nursing assessment, as well as goals and objectives related to health, nutrition, and safety.
• Provide services using a coaching model with infants and families. This will include home visits, and may include child groups and parent groups.
• Monitor developmental milestones and progress with parents/caregivers to inform ongoing need for direct service, assessment and/or referrals.
• Provide parent/caregiver education on health and safety issues during home visits or parent education groups.
• Complete progress notes, reports, assessments, and correspondence as appropriate.
• Document all information necessary for the upkeep of client files.
• Attend program functions for clients and the public.
