Qualifications

• Master’s degree required.
• Five years’ experience with at least three years in an acute inpatient setting preferred.
• DC RN license required.
• Current certification in Case Management or Utilization Management preferred.

Responsibilities

• Assess individual patient and family needs and contribute to care planning.
• Implement targeted interventions to improve outcomes and support continuity of care.
• Collaborate with multidisciplinary teams to coordinate services across care settings.
• Ensure follow-up care and community resource connections to meet patient needs.
• Navigate payer systems and manage discharge planning to optimize reimbursement and patient outcomes.
