Job summary

Seattle Children’s is seeking a dedicated nurse to coordinate care across patient populations from admission through discharge. You will advocate for patients, collaborate with families and the health care team, and promote efficient resource use and timely discharge planning.

The role requires a BSN degree and a WA nursing license, with a minimum of three years of clinical experience. BLS is required, with PEARS/PALS accepted as substitutes, and ongoing education support is provided.

Qualifications

• BSN degree or equivalent combination of education and experience.
• Minimum of three (3) years of recent clinical experience.
• Current Washington State Nursing License.
• Current BLS for Healthcare Providers is required for all positions; PEARS/PALS accepted in lieu of BLS.
• ACLS will not be accepted as a substitute.
• PALS certification may be required for specific departments.
• Case Managers in Child Psych Consultation must obtain Safety Care Certification within 3 months of hire and annually thereafter.

Responsibilities

• Coordinate care for specific patient populations from admission through discharge in collaboration with the health care team and family.
• Advocate for patients and families, ensure timely achievement of outcomes, and promote collaborative practice.
• Support staff/physician education related to the care coordination process.
