Job summary

Qualifications

• Minimum of 1 year of broad clinical experience with care coordination/discharge planning preferred.
• Working knowledge of third party payers' financial aspects encouraged.
• Familiarity with evidence-based guidelines to manage length of stay preferred.

Responsibilities

• Coordinate care to achieve optimal discharge and continuity of care outcomes.
• Establish estimated Length of Stay via MCG criteria and tools.
• Develop and implement a comprehensive discharge care plan with the clinical team.
• Communicate with payers to resolve potential denials and coordinate post-discharge needs.
