Job summary

Children’s National Hospital is seeking a Case Manager II to support patients and families across inpatient and emergency settings. You will coordinate services, assess needs, and drive care planning with a multidisciplinary team.

Lead efforts in discharge planning and resource linkage while advocating for patient-centered outcomes. A Masters in nursing and DC RN license are required; case management certification is preferred.

Qualifications

• Master’s Degree required.
• Five years’ experience with at least three years in an acute inpatient setting.
• RN license in DC required.
• Certification in Case Management or Utilization Management preferred.

Responsibilities

• Support patients and families throughout their healthcare journey with professional case management.
• Collaborate with multidisciplinary teams to coordinate services and care planning.
• Conduct assessments to identify discharge barriers and resource needs.
• Develop and implement plans of care and make referrals based on needs assessments.
• Engage in community outreach to connect families with essential resources and services.
