Job summary

PSCFL is seeking a Registered Nurse with expertise in discharge planning and case management to optimize patient transitions and outcomes. You will coordinate DME, home health, and post-acute services, collaborating with families and payors to ensure timely discharges.

Ideal candidates have Florida licensure and 1–3 years in discharge planning or transitions of care, along with strong communication skills in a fast-paced hospital setting.

Qualifications

• RN licensure in Florida (or multi-state compact) in good standing.
• 1–3 years pediatric or adult acute hospital care experience.
• 1–3 years in discharge planning, utilization/case management, or transition of care.

Responsibilities

• Coordinate DCP needs for pediatric patients including DME, home health, and rehab services.
• Screen charts for admission and level-of-care criteria; monitor plan of care.
• Perform reviews using InterQual/MCG criteria as applicable.
• Collaborate with care team to assess discharge needs and cost-benefit of plans.
• Identify barriers to discharge and coordinate timely care with stakeholders.
• Liaise with payors and vendors for authorizations and post-acute services.
• Document case management information accurately in systems.
• Participate in LEAN projects and interdisciplinary rounds.
