You will collaborate with the care team, utilize MCG criteria, and advocate for patient- and family-centered care across the continuum.

The role emphasizes independent work with supervision and requires strong communication with families, clinicians, and payers to ensure timely transitions and adherence to care plans.

Qualifications

• RN license or eligibility in Arizona
• Bachelor's degree in Nursing or equivalent with clinical experience in care coordination
• Experience with discharge planning and utilization review may be required

Responsibilities

• Manage a defined service line patient population to achieve optimal discharge and continuity of care outcomes.
• Establish estimated Length of Stay via MCG criteria and tools.
• Coordinate development and implementation of a comprehensive discharge care plan with the clinical care team.
• Identify and resolve barriers to timely discharge and coordinate referrals across disciplines.
• Communicate with patient/family and payers to ensure understanding and access to post-discharge services.
