Job summary

Nicklaus Children’s is seeking an experienced RN Case Manager in Fort Lauderdale, FL to coordinate discharge planning across pediatric and adult services. You will collaborate with the health care team to assess patient needs, ensure appropriate care levels, and engage payors for authorization to support timely discharges.

The role emphasizes utilization management, transition of care, and safe discharge, with strong communication to families and the clinical team.

Qualifications

• RN licensure in Florida or multi-state compact (eNLC) in good standing.
• 1–3 years pediatric or adult acute clinical hospital experience.
• 1–3 years in discharge planning, utilization management, case management, or transition of care.

Responsibilities

• Coordinate DCP needs for pediatric patients including DME, home health nursing, rehab, and other services.
• Screen charts to determine admission and level of care criteria; monitor daily plan of care.
• Perform admission and concurrent/retro reviews; refer for medical review when criteria not met.
• Collaborate with health care team to assess discharge needs and cost-benefit for discharge planning.
• Identify and resolve delays for safe discharge; seek consultations to expedite care.
• Communicate with payors to obtain authorization for services to support transition to appropriate care.
