Job summary

Nicklaus Children's Health System in Fort Lauderdale, FL is seeking a skilled RN with case management expertise to coordinate discharge planning and transition-of-care activities for pediatric and adult patients. You will collaborate with the health care team, payors, and families to ensure safe, cost-effective care transitions.

The role requires Florida RN licensure (or eNLC), 1–3 years in discharge planning/care coordination, and proficiency with Cerner and MS Office applications.

Qualifications

• RN licensure in Florida or Multi-State Enhanced Nursing License Compact (eNLC) in good standing.
• 1-3 years of pediatric or adult acute clinical hospital experience.
• 1-3 years of experience in discharge planning, utilization management, case management, performance improvement, managed care reimbursement, or transition of care.

Responsibilities

• Coordinates DCP needs for pediatric patients including DME and home health nursing and rehab services.
• Screens charts to determine admission and level-of-care criteria; monitors daily plan of care.
• Performs admission and concurrent reviews; refers for medical review when criteria aren’t met.
• communicates with care team to assess discharge barriers and applies cost-benefit analysis.
