Job summary

Nicklaus Children's Health System seeks an experienced Registered Nurse to lead discharge planning and transition of care initiatives for pediatric patients. You will collaborate with physicians, case managers, and social work to ensure safe, timely, and cost-effective discharge planning.

Ideal candidates will have Florida RN licensure, 1–3 years in discharge planning or case management, and familiarity with Cerner and post-acute care pathways.

Qualifications

• RN licensure in Florida or multi-state compact (eNLC) in good standing.
• 1-3 years of pediatric or adult acute clinical hospital experience preferred.
• 1-3 years of discharge planning, utilization management, case management, or transition of care experience preferred.

Responsibilities

• Coordinate DCP needs for pediatric patients, including DME, home health, and rehab services.
• Screen charts and monitor daily plan of care; collaborate to remove barriers to timely care.
• Perform admission and concurrent reviews using InterQual or equivalent criteria.
• Coordinate with health care team to assess discharge needs and cost-benefit considerations.
• Identify and resolve delays to safe discharge; escalate when needed to expedite care.
• Engage payor resources to secure prior authorizations for post-acute services.
• Communicate with families, payors, and providers to ensure appropriate resource use and placement.
