Formación

• Minimum 2 years in health care, care management or related setting.
• RN license active and unrestricted in state of practice.
• Valid driver’s license, reliable transportation for travel requirements.
• Understanding of EMR and HIPAA.
• Knowledge of community resources and strong communication.

Responsabilidades

• Completes comprehensive member assessments per timelines and identifies care management candidates.
• Develops and implements care coordination plans with member, caregiver and providers.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents progress toward goals.
• Maintains ongoing caseload with regular outreach and coordination of services.
• Promotes integration of behavioral health, LTSS, and community resources.
