Job summary

Molina Healthcare, Inc. is seeking a licensed behavioral health clinician to support care management and coordination across the continuum.

You will collaborate with a multidisciplinary team to ensure quality and cost-effective member care, and you will help drive outcomes through structured care plans and regular member contact. Responsibilities include comprehensive assessments, care plan development, home visits as needed, and ongoing monitoring.

Qualifications

• At least 2 years health care experience, preferably behavioral health.
• Active license as one of the listed behavioral health clinicians.
• Valid driver's license and reliable transportation for travel.
• Experience with severe mental health concerns and SUD, foster care.
• Knowledge of whole person care, chronic conditions, discharge planning.
• Data entry and experience with a clinical platform.
• Excellent verbal and written communication.

Responsibilities

• Completes behavioral health assessments within regulated timelines.
• Develops and implements care plans with the member and team.
• Conducts telephonic, face-to-face or home visits.
• Monitors and documents care plan effectiveness and updates as needed.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health with LTSS and community resources.
• Facilitates care team meetings and ICT collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides coordination.
• May provide consultation and resources to peers.
• 25-40% local travel may be required.
