Job summary

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Active license: LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD or PsyD as applicable.
• Valid driver’s license and reliable transportation for travel.
• Experience with severe mental health concerns and substance use disorders.
• Knowledge of whole person care and discharge planning coordination.
• Data entry skills using a clinical platform.
• Excellent verbal and written communication.
• MS Office proficiency.

Responsibilities

• Provides care management and care coordination with multidisciplinary team.
• Develops and implements care plans with member, caregiver, physician, and other professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Performs ongoing monitoring of care plan and documents progress toward goals.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services and resources to enhance continuity of care.
• Facilitates interdisciplinary care team meetings and informal ICT collaboration.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation and resources to peers as needed.
• 25-40% estimated local travel may be required.
