Job summary

Qualifications

• RN license in state of practice.
• BLS required within 6 months of hire.
• Preferred: 1–2 acute care/pediatrics/specialty case management.
• Knowledge of care coordination models and regulatory guidelines (CMS, URAC).
• Certifications CCM and ACM are advantageous.
• BSN degree preferred.

Responsibilities

• Coordinate comprehensive assessments to identify patient and family needs.
• Develop individualized, goal-driven care plans with the interdisciplinary team.
• Identify logistical needs (travel, housing) to facilitate access to care.
• Balance resources with patient preferences to support informed decisions.
• Support education and training for the interdisciplinary team.
• Utilize evidence-based practice to optimize patient outcomes.
