Job summary

Shriners Children’s is seeking a Care Manager to coordinate and facilitate comprehensive, patient-centered care for children with complex medical needs, ensuring optimal outcomes and a seamless healthcare experience for patients and families.

The role requires an active RN license in the state of practice and BLS within 6 months of hire. The position offers opportunities across Shriners Children’s facilities in the U.S. with a dedicated interdisciplinary team.

Qualifications

• RN license in state of practice.

• Experience in care coordination is preferred.

Responsibilities

• Perform comprehensive assessments to identify patient and family needs, early in the care process, to ensure smooth progression throughout the continuum of care.
• Identify logistical needs, including travel, housing, and other non-clinical resources to facilitate access to care and refer to appropriate support staff.
• Develop individualized, goal driven care plans in collaboration with the interdisciplinary team to optimize patient outcomes.
• Balance available resources with patient preferences, to facilitate informed decision making.
• Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to support appropriate resource management.
• Support interdisciplinary team education and training.

Job Summary

Required Qualifications

• Basic Life Support (BLS) Required within 6 months of hire

Preferred Qualifications

• 1-2 Acute Care/Pediatrics/Specialty Case Management
• Preferred Knowledge of care coordination models and regulatory guidelines (e.g., CMS, URAC) CCM, ACM BSN

Compensation

Compensation is determined based on years of relevant experience and departmental equity.
