Job summary

Shriners Children's is seeking a Care Manager to coordinate and facilitate comprehensive, patient-centered care for children with complex medical needs, ensuring optimal outcomes and a seamless healthcare experience for patients and families.

The role involves working with the interdisciplinary team to identify needs, develop individualized care plans, and balance resources with patient preferences while supporting education and training across the care continuum.

Qualifications

• RN license in state of practice.
• BLS certification within 6 months of hire.
• Preferred qualifications: 1-2 Acute Care/Pediatrics/Specialty Case Management.
• Preferred knowledge of care coordination models and regulatory guidelines (e.g., CMS, URAC) CCM, ACM BSN.

Responsibilities

• Perform comprehensive assessments to identify patient and family needs, early in the care process, to ensure smooth progression throughout the continuum of care.
• Identify logistical needs, including travel, housing, and other non-clinical resources to facilitate access to care and refer to appropriate support staff.
• Develop individualized, goal driven care plans in collaboration with the interdisciplinary team to optimize patient outcomes.
• Balance available resources with patient preferences, to facilitate informed decision making.
• Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to support appropriate resource management.
• Support interdisciplinary team education and training.

Compensation is determined based on years of relevant experience and departmental equity.
