Responsibilities

• Advocates for patient and family within treatment plan and guidelines.
• Collaborates with interdisciplinary team to meet facility goals.
• Performs intake assessments at initial visit or by phone.
• Coordinates care with Medical Home and other health care providers.
• Provides discharge/transition assessments and identifies barriers.
• Facilitates medication management and adherence with proactive follow-up.
• Reviews service authorizations to support medical necessity and level of care.
