Pediatric Care Manager - Behavioral Health

Join to apply for the Pediatric Care Manager - Behavioral Health role at Sunshine Health.

Position Purpose: Develops, assesses, and facilitates complex care management activities for members with mental and behavioral health needs to ensure high-quality, cost-effective healthcare outcomes. Responsibilities include creating personalized care plans, providing education to members and families, and coordinating care involving behavioral health and social determinants.

This is a Remote/field-based position; candidates must reside in Lee County for face-to-face visits.

Responsibilities:
  1. Evaluate member needs via phone or in-home visits, and develop appropriate care or service plans.
  2. Conduct outreach through telephonic, digital, or home visits to assess needs and collaborate with resources.
  3. Create ongoing care plans for high-acuity members, identifying providers and community resources.
  4. Coordinate services between members, families, community resources, and care teams.
  5. Monitor care plans and member outcomes, adjusting as needed based on clinical changes.
  6. Facilitate timely access to care by collaborating with providers and specialists.
  7. Maintain accurate documentation of all care management activities in compliance with regulations.
  8. Educate members and families on procedures, treatment options, and healthcare benefits.
  9. Provide feedback to leadership for continuous improvement of care delivery.
  10. Perform other duties as assigned, adhering to all policies and standards.
Qualifications:
  • Master's degree in Behavioral Health, Social Work, or Nursing from an accredited institution.
  • 2-4 years of related experience.
  • Licensed Behavioral Health Professional or RN, with specific licenses such as LCSW, LMSW, LMFT, LMHC, LPC, or RN with behavioral health experience, depending on state requirements.

Pay range: $55,100 - $99,000 annually. Benefits include health insurance, 401K, stock plans, tuition reimbursement, paid time off, and flexible work arrangements.
