Base Pay Range

$53,045.00/yr - $66,837.00/yr

Role Overview

Join our integrated care team to support children with diverse medical and behavioral health needs, including intellectual/developmental disabilities, mental health issues, and complex medical conditions.

Key Responsibilities
  • Advocate and educate continuously.
  • Develop assessments and care plans, including crisis plans and evaluations.
  • Collaborate with members and the care team.
  • Update assessments and plans regularly.
  • Provide resources and monitor quality care within established timeframes.

Qualifications

Education: Bachelor's, Master's, or licensed HS professional/RN in NC.

Preferred: Experience in Care Management, Service Coordination, Mental Health Counseling, working with dually diagnosed children.

Licenses/Certifications: Valid NC driver’s license required. RN license preferred if applicable.
