Humana Healthy Horizons in Kentucky is seeking a Field Care Manager, Behavioral Health 2 who will assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness. The role involves guiding members and families toward appropriate resources for care and wellbeing. The work assignments are varied and often require independent interpretation and decision-making.
Care Management is an approach to managing enrollees’ care needs, aiming to improve patient care and reduce medical services by enhancing coordination, eliminating duplication, and empowering patients and caregivers.
Role Responsibilities
- Coordinate and collaborate with providers and community services to promote quality and cost-effective outcomes.
- Manage cases involving individuals identified through the Health Needs Assessment.
- Employ strategies to manage physical, environmental, and psycho-social health issues by advocating, empowering, and assisting members.
- Identify and resolve barriers and social determinants affecting care and health outcomes.
- Monitor patient progress through assessments and evaluations to ensure desired outcomes.
- Develop care plans and influence departmental strategies and case approaches.
- Exercise autonomy in setting objectives and approaches; maintain knowledge of community health and social services.

Required Qualifications
- One of the following: active KY Licensed Masters Clinical Social Worker (LCSW), KY Licensed Professional Clinical Counselor (LPCC), KY Licensed Marriage Family Therapist (LMFT), other KY independent license with at least 1 year of clinical experience, or active KY Registered Nurse (RN) license with at least 3 years of behavioral health experience.
- Proficiency in Microsoft Office Suite and ability to troubleshoot technical issues remotely.
- Experience with Severe and Persistent Mental Illness (SPMI) and/or Substance Use Disorder (SUD).
- Experience with behavioral change, health promotion, coaching, and wellness.
- Reside in Kentucky or a contiguous state (Ohio, Indiana, Tennessee, Virginia).
Preferred Qualifications
- Experience with Kentucky Medicaid population.
- Managed Care experience.
- Case Management Certification (CCM).
- Bilingual in English and Spanish.
Additional Information
- Workstyle & Travel: Remote position with up to 10% travel within 50 miles. Includes face-to-face and virtual meetings.
- Driver's License & Insurance: Valid license, insurance, and reliable vehicle required.
- Schedule: Monday-Friday, 8:00 AM - 6:00 PM EST, flexible within this window.
- Language & TB Screening: Language proficiency assessment required; TB screening mandatory due to patient-facing role.
- Work from Home Requirements: Internet speed of at least 25 Mbps download and 10 Mbps upload; dedicated workspace necessary.

- Travel: Occasional travel for training or meetings.
- Hours & Compensation: 40 hours/week; $59,300 - $80,900 annually; eligible for bonuses.
