Job summary

Shriners Children’s is seeking an RN Care Manager to provide assessments, planning, interventions, and follow-up for pediatric patients and families across the continuum of care. You will coordinate plans of care, ensure safe and effective care, and document assessments and outcomes in accordance with SHC policies and regulatory requirements.

Qualifications

• Possess a BSN and a current RN license in the state of practice.
• Minimum 3 years of nursing experience.
• Experience in pediatrics, burn care, or orthopedics preferred.

Responsibilities

• Coordinate the plan of care to achieve optimal patient outcomes.
• Identify patient needs early to develop a safe, effective care plan.
• Collaborate with patients, families, and the health care team across the SHC system.
