Qualifications

• BSN with disease management experience and motivational interviewing techniques.
• RN license in the state of practice.
• Experience with pediatric care coordination is preferred.

Responsibilities

• Advocates for patients and families within treatment plans and UR/UM guidelines.
• Collaborates with interdisciplinary teams to meet organizational goals.
• Performs intake assessments at initial visit or by phone prior to first appointment.
• Ensures coordinated care with Medical Home or other health care teams.
• Provides discharge/transition assessments and plans to reduce barriers to outcomes.
• Supports medication management and adherence through proactive follow-up.
• Reviews service authorizations and documentation for medical necessity.
